Thalakkulathur Mathilakam temple is located at Thalakulathur in Kozhikode district, Kerala. The temple is also known as Sri Narayana Perumal or Thalakkulathur devaswom. The shrine, which is more than 1500 years old, is dedicated to Narasimha Avatar of Bhagwan Vishnu. Narasimha Jayanti in Vaishaka Masam (May) is the most important festival in the temple. The sankalpam of Narasimha here is that of Lakshmi Narasimha. The temple has a two-tier chathura sreekovil, namaskara mandapam, nalambalam and shrines of upa devatas. The upa devatas worshipped in the temple are Shiva, Sastha, Ganapathy and Goddess Bhagavathy. Bhagavathy is worshipped in the Kannadi Bimbam form here. It is believed that the shrine of Bhagavathy existed here before all the other shrines. As per local legend, the origin of the temple is traced backed to Agastya Muni of Treta Yuga.
