Siddheshwar Mahadev is worshipped in Ujjain in
Madhya Pradesh. It is one of the 84 forms of Shiva worshipped in Ujjain.

near an ancient pipal tree.

Story of Siddheshwar Mahadev
Long ago lived a king named Ashvishwara. The subjects of the
kingdom were happy to have a king who followed Dharma. He had performed
numerous sacrifices and attained various Siddhis.
Once Saint Kapila and Saint Jaigashtya visited the king. The
king wanted to know about creation and role played by Vishnu. The two saints
then showed creation and role of Vishnu in the palace through their Siddhis.
The king then wanted to know how to get such Siddhis.
The two saints asked the king to go to Ujjain and perform prayers
to Siddheshwar Mahadev.
The king went to Ujjain and offered prayers to Siddheshwar
Shivling. The king was able to see Vishnu seated in the middle of the Shivling.
The king realized that there is only one Supreme Truth known
by various names.
The king had self-realization and attained moksha.
