Azhikode Myladathadam Vannarath Puthiya Bhagavathy temple is located at Myladathadam near Azhikode in Kannur District in Kerala. The temple is dedicated to Goddess Bhagavathi. The annual festival at Myladathadam Vannarath Puthiya Bhagavathy temple is observed in Meena Masam (March - April) – Meenam 28 to 30 (April 11 or April 12 to April 13 or April 14).
The theyyams performed in the temple are Gulikan Theyyam, Puthiya Bhagavathy, Veera Kali Theyyam and Veeran Theyyam.
This is a small kavu temple with a chathura sreekovil. Bhagavathy here is unique as she wears garlands as head dress.
