Story of Kakotishwar Mahadev form of Shiva in Ujjain
Legend has it that once Goddess Uma cursed the nagas that they will perish at the yajna of Janmajeya. Some Nagas led by Karkotaka approached Brahma who advised them to go to Mahakaal Vana and offer prayers to Lord Shiva.

Here, the nagas made a Shivling and offered prayers. Pleased with their devotion, Shiva gave the boon to Karkotaka that those nagas that followed Dharma would be rescued.

As Karkotaka offered prayers here, the Shivling came to be known as Kakotishwar Mahadev.
